By signing up for a class or service you hereby voluntarily participate in the activities, classes, and services offered by Minicucci Pilates, located at 7494 Santa Monica Blvd. Suite 202, West Hollywood, CA 90046. I understand that these activities may include but are not limited to Pilates exercises, movement classes, and fitness training.
In consideration of my participation in these activities, I hereby release, discharge, and hold harmless Minicucci Pilates, its instructors, employees, and any affiliated entities, including David Minicucci, from any and all claims, liabilities, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or harm that may occur as a result of my participation.
I understand and acknowledge the potential risks associated with physical activity and movement exercises. I confirm that I am in good health and have no medical conditions that would prevent me from safely participating in these activities. If I have any concerns about my health, I will consult a medical professional before participating.
**Note Regarding Images and Photographs:**
By signing up for a class or service you hereby grant Minicucci Pilates, including David Minicucci, the permission to use any photographs, videos, or images taken during my participation in their activities for promotional, marketing, or educational purposes. I understand that my name, likeness, and image may be used in printed materials, social media, and on the Minicucci Pilates website.
